Chronic Desease Management
The Practice Nurses offer Chronic Disease Management clinics for patients diagnosed with Asthma, Chronic Obstructive Pulmonary Disease (COPD), Heart Disease, Hypertension, Chronic Kidney Disease, Stroke and Diabetes. Other services available from the practice nursing team include:
- Dressings, suture removal, ear syringing.
- Travel advice, yellow fever vaccination, flu jabs, injections.
- Well person checks, cervical smears, smoking cessation advice, dietary advice.
- Venepuncture.
- Memory checks (MMSE).*
- Doppler studies.*
- ECG’s.*
- Spirometry.*
- Cryotherapy (wart) clinics.*
Nursing services are available by appointment. (*Some services require GP referral).
