New Patient Registration
If your are resident within the designated practice area (see location map), you can register with the practice.
If you would like to register with this practice then please collect a New Patient Pack from the Health Centre. You will be asked to complete an official NHS registration form for each person or family member registering. You will also be given a new patient medical questionnaire and an ethnicity form, which provides us with basic medical information for the period until your notes reach the practice. Please bring your NHS medical card with you when collecting your New Patient Pack.
When registering you will also be asked to provide two proofs of identity, one of which should show your address, and one should show photographic identity.
You will be asked to make an initial appointment with the Healthcare Assistant. We would encourage you to keep this new patient medical.
It is our policy that patients register with the practice and not with an individual GP. Your medical card will have a Doctor's name attached, but this is purely for administrative purposes. You may indicate your preference to see or speak to a particular Doctor, but the practice cannot always accommodate these requests as specific Doctors may not always be available.
